Our Transitions Hub supports young people across the Liverpool City Region who have an Education, Health and Care Plan (EHCP) and are working at Entry Level 2 and above, helping them prepare for adulthood, further education, or employment.

The Transitions Hub is for learners aged 16–25 from Sefton, Liverpool, Wirral, Halton, St Helens, and Knowsley who:

• Are working at Entry Level 2 and above
• Have an EHCP (Education, Health and Care Plan)
• Would benefit from a smaller, more supported environment
• Have social, emotional and mental health needs that are currently a barrier to them accessing general further education
• Are aiming for future progression into college, employment, or supported internships
• Need to build confidence, soft skills, and independence
